Repeat Prescriptions

We are happy to take requests for repeat prescriptions in person, by letter,by fax or you can order via our online services.

How to request a repeat prescription

• Please use the computer slip attached to your previous prescription.
  
• Please tick the appropriate boxes on this slip.
• You may either send the computer slip by post (The Callowland Surgery, 141a Leavesden Road, Watford, WD24 5DG) or alternatively,
• Fax the details from the presciption print out to the Practice Fax No: 01923 819047

Please allow two working days from the Practice's receipt of your request for your prescription to be processed.

Your doctor may wish to see you from time to time to review your medication.

For security reasons we are not able to take telephone requests for repeat prescriptions.
